Placenta Encapsulation Health Affidavit & Consent. For the health and safety of all clients, I require confirmation that your placenta is free of certain transmissible infections. The following declarations serve in lieu of submitting bloodwork.

I affirm that I have not tested positive for HIV (Human Immunodeficiency Virus).
I affirm that I have not tested positive for Hepatitis B or Hepatitis C.
I understand that if I have tested positive for any of the above infections, my placenta is not eligible for encapsulation due to safety risks
I acknowledge that testing positive for GBS (Group B Streptococcus) does not disqualify my placenta from encapsulation. I understand that GBS is a common colonization, not an infection, and that when the placenta is prepared following safe food handling and dehydration guidelines, it does not pose a risk.
I agree to notify my encapsulator immediately if I am diagnosed with any communicable disease that could affect the safety of my placenta products.
I understand that these services are not a substitute for medical care.
I agree to clearly label my placenta storage container with my full name, date of baby's birth, and contact phone number prior to pickup or drop-off. I understand this is required for proper identification and safe handling
